Bioidentical hormones are chemical copies of human hormones, absolutely received by the body as natural: they have the exact molecular structure of those synthesized in the human body and produce the same physiological responses as endogenous hormones.

Often my patients get a little confused when we talk about bioidentical hormones and say to me, but these are better because they are natural. But what does "natural mean"????

Bioidentical hormones (OB) have the same structure as the hormones in our body, unlike synthetic ones, and are extracted from plants (such as Dioscorea Villosa, soybean, etc.): therefore, our body recognizes them as its own.
However, these are hormones in their own right, not simply herbal medicines. They should always be prescribed by a physician, after specific examinations and in case of actual deficiency, and treatment should be monitored, because each individual case differs from another. Their effect is less potent, so much so that Are not effective as contraceptives. The body recognizes them and therefore tolerates them better, without those side effects that often cause people to discontinue treatment with synthetic hormones, because OBs we can dose them according to deficiency and clinical picture.

But what are the identical hormones? 

The most widely used are those lacking in pre- and perimenopause and menopause such as progesterone, estradiol, estriol, DHEA (dehydroepiandrosterone) but also pregnenolone, cortisol, testosterone, and thyroid hormone.

The goal of bioidentical hormone replacement therapy is to alleviate symptoms caused by the body's reduced production of hormones and reduce the incidence of disease related to hormone imbalances by promoting balance and mimicking, as much as possible, the body's natural processes. Dosage should be individualized and determined by a physician competent in bioidentical hormone therapy: I discourage any foma of do-it-yourself and ordering products on the internet! 

It is important to take the physiologic dosage of the missing hormone, correct any other concomitant hormone deficiencies, start the treatment at the right time, with the minimum dosage needed, and perform regular checks over time, through laboratory tests and their correct interpretation, evaluating the symptoms, signs of deficiency, excess or imbalance of hormones or groups of hormones, taking into account each individual's family history and response to previous drug therapies. On this is based the right dose or rather the physiological i.e. optimal dose of the hormone, depending on the presented disorder, gender and age. For this the saliva test, which determines the fraction of the free hormone i.e., corresponds to the tissue hormone concentration, is very suitable. Blood tests are, of course, useful: however, we must keep in mind that they often have an overly broad and nonspecific laboratory normality range.
